SBA Information Notice TO: All SBA Employees, and Certified Development Companies CONTROL NO.: 5000-881290 SUBJECT: Renewal of SBA Form 1081, Statement of Personal History EFFECTIVE: September 1, 2026 The purpose of this Notice is to announce the release of the updated SBA Form 1081, Statement of Personal History, which expires April 30, 2029. The content and instructions remain the same, only the expiration date has changed to renew the form. The renewed SBA Form 1081 is effective immediately; however, SBA will allow a 30-day grace period from the date of this SBA Information Notice for Certified Development Companies (CDCs) to begin using the revised form. Until then, CDCs may use the previous version of the form. After the grace period ends SBA will no longer accept the previous version. Questions concerning this Notice may be directed to the Lender Relations Specialist in the local SBA Field Office. Daniel J.
